Job Description

Job Summary:

Coordinates the overall interdisciplinary plan of care for patient, from admission to discharge.

Responsibilities:

  • Creates plan for care across the continuum, integrating patient/family preferences and values.
  • Monitors patient care through assessments, evaluations and/or patient records.
  • Advocates for resources and removal of barriers.
  • Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
  • FMC - Assesses patients and assists with developing care plans for patients.
  • FMC - Provides hands-on clinical assistance as needed to complete care coordination tasks.
  • FMC - Familiar with population health management strategies, care coordination activities and caring for patients with preventative and chronic care gaps.
  • FMC - Utilizes data analysis and tracking tools to manage population health patients.


Qualifications

Licenses/Certifications/Registration:

  • Required Credential(s):
    • One or more of the following:
      • Licensed Advanced Practice Nurse Practitioner credentialed from the Florida Department of Health obtained prior to hire date or job transfer date.
      • Licensed Registered Nurse credentialed from the Florida Department of Health obtained prior to hire date or job transfer date.

Education:

  • Associate Degree
    • Graduate of an accredited school of professional nursing required
    • ASN or BSN preferred

Work Experience:

  • Neuro Support - 5+ years nursing experience required; experience in orthopedic, spine and/or program development preferred.Family Medicine - 5+ years nursing experience required; experience in family medicine, primary care, med-surg and/or program development preferred
